Beginning 9-1-23: (and every September thereafter, we ask that you go into your portal and resubmit all consent forms plus the Office policies form (this will be a total of 6 forms). They are all electronically signed and you can find them under the Questionnaires tab, then scroll down to the Consents for ALL Patients section.
Office Updates:
12/25 to 1/3 - we will be out of the office and will return on Thurs 1/4/24
Normal Business Hours:
Office Hours for DR DAM and for NURSE MAIKA are Monday and Thursday 9 -5p - all questions, messages and refills requests are handle on these days
